While it is not required, please let us know you are coming by Oct. 2, 2024 so we can be responsible with vaccine supply.

 

Completing the form does not guarantee you a vaccine. Shots will be administered to any currently enrolled students on a first-come, first-serve basis.

We will have these forms available at the flu shot clinic. You can also complete the form and bring the printed document to the flu shot clinic. If you are a minor, please bring the completed form with your guardian's signature.
